Stephen Douglas was stabbed to death by a mental health client in 2019, just one year after a similar, but non-fatal, stabbing at his workplace.

Stephen's family say his death was preventable, as a coroner found "inadequacies" in decision-making and "unreasonable" workloads at his workplace.
